Novartis's New MS Pill Doesn't Nuke Your Immune System. It Just Turns Down the Heat.

Most MS drugs work like bouncers: they block, remove, or deplete immune cells entirely. Novartis's remibrutinib takes a different approach, dialing down the overactive enzyme (BTK) that tells immune cells to attack the brain, without wiping them out. Across two Phase 3 trials with roughly 2,000 patients, the oral pill beat teriflunomide on relapses, shrank inflammatory brain lesions, and showed hints of slowing disability progression at six months. Crucially, there were zero liver toxicity signals, a problem that has tripped up rival BTK inhibitors. Analysts project peak sales of $3 billion to $5 billion or more.

Why it matters: This is the strongest Phase 3 readout yet for a BTK inhibitor in MS, validating a new drug class that could calm neuroinflammation from inside the brain rather than just at its borders. It reshuffles a four-way competitive race with Sanofi, Roche, and Merck KGaA in a $27 billion market.
