Roche's fenebrutinib has shown broad efficacy signals across both relapsing and primary progressive MS, positioning it as the potential best-in-class contender. Merck KGaA's evobrutinib, meanwhile, stumbled in Phase 3 back in 2023 and has fallen behind.
Remibrutinib's entrance reshuffles the deck. Its clean safety profile and strong relapsing MS data give it a distinct competitive angle: a potentially best-in-class oral option for the largest slice of the MS market.
And that market is enormous. The global MS therapeutics space is worth roughly $27 billion, with the relapsing segment alone accounting for a huge chunk. Existing treatments include injectable antibodies like Ocrevus, oral S1P modulators like Mayzent, and older pills like teriflunomide. An effective, well-tolerated oral BTK inhibitor could carve out significant share by offering patients something genuinely new.
Skeptical readers might notice that Novartis chose to compare remibrutinib against teriflunomide, which is considered a moderate-efficacy MS drug. It's a fair question: why not test against one of the big guns like Ocrevus?
The honest answer is that teriflunomide is a common comparator in MS trials because it's oral (matching the route of administration) and because regulatory agencies accept it as a reasonable benchmark. Beating teriflunomide gets you in the door. The real test of remibrutinib's ceiling will come from head-to-head data or real-world comparisons against high-efficacy therapies.
