

Pfizer just recalled the only FDA-approved drug that can treat syphilis in pregnant women and protect their babies. With congenital syphilis cases up 700% since 2015 and supply already strained for years, the timing couldn't be worse.
Imagine you're a pregnant woman diagnosed with syphilis. There is exactly one drug on the planet that can treat your infection and protect your baby. Not two options. Not three. One.
Pfizer just recalled a bunch of it.
Pfizer issued a voluntary recall of Bicillin L-A, the only FDA-approved benzathine penicillin G product sold in the United States. The company found particulate matter during visual inspection of certain lots and traced the contamination to stoppers from an outside vendor. The recalled lots cover both the 1.2 million unit and 2.4 million unit prefilled syringes, which are the two workhorses of syphilis treatment in clinics across the country.
Pfizer classified the risk to patients as "medium" and said no adverse events had been reported. No signs of reduced effectiveness either. But that's not really the problem.
The problem is that there's almost nothing left on the shelf. Pfizer warned of a near-term stockout of both strengths and said it would begin rationing inventory on a per-patient basis. When your only supplier starts using the word "ration," you know things are bad.
If this feels like déjà vu, that's because Bicillin L-A has been in short supply since spring 2023. The FDA first flagged the shortage in April of that year, citing increased demand as syphilis cases surged nationwide. Pfizer, the sole U.S. manufacturer, simply couldn't keep up.
The FDA tried to plug the gap by temporarily importing foreign benzathine penicillin products like Extencilline (authorized in January 2024) and Lentocilin. Those imports helped, but they were Band-Aids on a wound that kept getting bigger. Supply recovery was originally expected by now; it's been pushed back to late 2027.
Then in 2025, Pfizer issued another voluntary recall for particulate contamination, tightening supply even further. This latest recall layers yet another crisis on top of an already strained system. It's like running out of gas on the highway, getting a flat tire, and then watching it start to rain.

For most infections, doctors have a menu of antibiotic options. Allergic to one? Try another. That menu doesn't exist for syphilis in pregnancy.
Benzathine penicillin G is the only antibiotic proven to treat maternal syphilis and prevent transmission to the fetus. That's not a preference; it's a hard biological fact. The CDC's guidance is unambiguous: pregnant patients with syphilis get penicillin. Period.
What about alternatives? Doxycycline, the main backup for non-pregnant patients, is avoided during the second and third trimesters. Erythromycin and azithromycin don't reliably cure the infection or protect the fetus. The CDC specifically says they shouldn't be used. Even if a pregnant patient is allergic to penicillin, the official recommendation is desensitization (gradually building tolerance) followed by penicillin treatment. Not a switch to a different drug. There is no different drug.
This makes the supply situation feel less like a logistics headache and more like a public health emergency.
Congenital syphilis (when a mother passes the infection to her baby during pregnancy) has been climbing in the U.S. for 12 consecutive years. In 2024, the CDC reported nearly 4,000 cases, a roughly 700% increase from 2015.
The maternal syphilis rate jumped 28% between 2022 and 2024, rising from 280.4 to 357.9 per 100,000 live births. The increase cut across virtually all racial groups and age brackets. Interestingly, adult primary and secondary syphilis cases actually dropped 22% in 2024. But congenital syphilis kept climbing, which suggests the healthcare system is failing to screen and treat pregnant women before the infection reaches their babies.
Now remove the only effective treatment from the equation. The math gets terrifying fast.
The CDC's response has been straightforward: save every available dose of Bicillin L-A for pregnant patients and infants. For men and non-pregnant women with syphilis, clinicians should switch to doxycycline (100 mg twice daily for 14 days for early syphilis, 28 days for late or unknown-duration infections).
State health departments in Alabama, Washington, and elsewhere have echoed this approach, telling providers to monitor their supply closely and plan for an extended shortage. Some facilities are accessing imported benzathine penicillin products like Lentocilin as a substitute, but these remain the same drug class, just from a different source. They're not a new solution; they're the same solution with a foreign label.
Pfizer, for its part, said it's working to resolve the contamination issue and replenish stock. But public health officials aren't waiting around for good news. The guidance is clear: conserve, prioritize, and prepare for this to last a while.
This situation exposes a fragile truth about America's pharmaceutical supply chain. A single company makes the only approved version of a drug that is, for an entire patient population, medically irreplaceable. When that company stumbles, there's no backup plan beyond importing products from overseas under emergency authorization.
Penicillin isn't some exotic new biologic. It's a drug we've had since the 1940s. Alexander Fleming would be stunned to learn that, 80+ years after his discovery, the world's richest country can't reliably keep it in stock.
The recall itself may be routine. Particulate contamination happens; companies catch it, pull the lots, fix the problem. But routine quality events become public health crises when the supply chain has zero slack. One manufacturer, one product, rising demand, and a patient population with no therapeutic alternatives: that's not a supply chain, it's a tightrope.
For pregnant women diagnosed with syphilis this month, the question isn't abstract. It's whether their pharmacy has the only drug that can protect their baby. And right now, the honest answer is: maybe not.
