

Vera Therapeutics' Trutakna posted a 76% reduction in kidney disease progression with zero patients needing dialysis or transplant over two years. Those are the kind of numbers that make nephrologists do a double-take, and the IgA nephropathy treatment landscape may never look the same.
In clinical trials, you learn to celebrate modest wins. A 20% improvement here, a statistically significant nudge there. You pop champagne for a hazard ratio of 0.7. So when Vera Therapeutics announced that its kidney drug Trutakna cut the risk of disease progression by 76% over two years, people noticed. When the company added that zero patients on the drug experienced hard kidney outcomes like dialysis or transplant (compared to eight on placebo), the reaction shifted from interest to disbelief.
Those are the kinds of numbers that make seasoned nephrologists squint. Let's talk about why they might be real, what they mean, and why IgA nephropathy patients just got their most hopeful headline in years.
IgA nephropathy (IgAN) is the most common form of glomerulonephritis worldwide, which is a fancy way of saying it's the most common type of kidney inflammation caused by the immune system. Your body produces a defective version of an antibody called IgA1, and those broken antibodies clump together and lodge in the tiny filters of your kidneys. Over time, this causes scarring, protein leaking into your urine (proteinuria), and a slow, grinding loss of kidney function.
The cruelest part? It's a slow burn. Many patients are diagnosed in their 20s and 30s, then spend decades watching their kidney function decline. Think of it like termites in a house: you can't see the damage from the outside, but one day the floor gives out. Up to 40% of patients eventually need dialysis or a transplant.
For years, the only real treatment was blood pressure drugs (ACE inhibitors and ARBs) that reduce the strain on kidneys without addressing the root cause. Recently, SGLT2 inhibitors (originally diabetes drugs that happen to protect kidneys) and sparsentan (a dual-action drug targeting blood vessel constriction and blood pressure pathways in the kidney) have expanded the toolkit. But the core problem, the rogue immune system pumping out defective antibodies, remained largely untouched.
That's where Trutakna comes in.

Trutakna (atacicept) takes a fundamentally different approach. Instead of managing downstream damage, it goes upstream to the source of the problem.
Your immune system relies on two signaling proteins called BAFF and APRIL to keep B cells (a type of white blood cell) alive and active. B cells are the factories that produce antibodies, including the defective IgA1 antibodies that wreck kidneys in IgAN. Trutakna is a fusion protein that acts like a molecular sponge: it soaks up both BAFF and APRIL before they can reach B cells, effectively dimming the lights in the antibody factory.
Less signaling means fewer activated B cells. Fewer B cells means less defective IgA1. Less defective IgA1 means fewer immune complexes gumming up your kidney filters. It's an elegant chain reaction, and the clinical data suggests it actually works.
Importantly, this isn't a blunt-force immunosuppressant that leaves you vulnerable to every cold on the subway. It's a targeted B-cell modulator aimed specifically at the upstream drivers of IgAN. Think of it as a precision strike rather than carpet bombing.
The ORIGIN 3 study was a global, randomized, double-blind, placebo-controlled Phase 3 trial. That's the gold standard of clinical evidence: 428 adults with biopsy-confirmed IgAN were randomly assigned to either Trutakna (150 mg weekly injection) or a matching placebo for 104 weeks. All patients were already on maximum-tolerated standard kidney-protective therapy, so Trutakna wasn't replacing their existing drugs; it was being tested on top of them.
The results from the final analysis, reported in September 2026, were striking across every measure Vera pre-specified.
The headline: Trutakna reduced composite kidney disease progression events by 76% compared to placebo, with a hazard ratio of 0.24 (meaning patients on the drug had roughly one-quarter the risk of worsening). Only 11 events occurred in the Trutakna group versus 38 on placebo, and the statistical significance was overwhelming (p<0.0001).
But the statistic that really turned heads was the hard outcomes count. Among patients taking Trutakna, zero needed dialysis for 30 days or more, zero received a kidney transplant, and zero died from kidney-related causes. On the placebo side? Eight patients hit those devastating milestones.
Zero versus eight. In a 428-patient trial over two years, that's the kind of separation that makes you double-check the spreadsheet.
The kidney function data told a consistent story. At one year, patients on Trutakna lost essentially no kidney filtration capacity: their eGFR (a standard measure of how well kidneys filter waste) dropped by just 0.1 mL/min/1.73m². Placebo patients lost 5.7 over the same period. That's a placebo-adjusted benefit of 5.6 points, which is enormous in a disease where every point of eGFR represents months or years before dialysis.
Over the full two years, the annualized eGFR slope showed a placebo-adjusted benefit of 5.0 mL/min/1.73m² per year. To put that in context, a healthy person's eGFR naturally declines by about 1 point per year due to aging. Trutakna essentially froze kidney decline in its tracks while placebo patients continued their downward slide.
The drug also delivered statistically significant improvements in proteinuria (protein in the urine, the most visible sign of ongoing kidney damage), galactose-deficient IgA1 levels (the actual broken antibodies causing the disease), and hematuria (blood in the urine). Every prespecified endpoint was met.
A 76% risk reduction is an unusually large effect size for a chronic kidney disease trial. In this field, you're often celebrating 30% or 40% reductions. So it's fair to ask: is this too good?
A few things to consider. First, the trial enrolled patients who were already on optimized background therapy, which means the effect isn't confounded by undertreated controls. Second, the 104-week duration is meaningful; this wasn't a short sprint designed to cherry-pick early results. Third, the consistency across endpoints (eGFR, proteinuria, Gd-IgA1, hard outcomes) makes it harder to dismiss as a statistical fluke.
That said, 428 patients is a moderate-sized trial. The hard outcomes comparison (zero vs. eight) is compelling but based on small absolute numbers. And kidney disease is a decades-long story; two years of data, while encouraging, doesn't guarantee lifelong benefit. The ongoing 52-week open-label extension should provide additional durability data.
The drug also has a two-decade clinical development history that includes a safety stumble in multiple sclerosis (an unexpected increase in disease activity led to that program's suspension). Different disease, different context, but it's a reminder that atacicept's biology is powerful and needs to be wielded carefully.
Vera's stock jumped 13% after the data dropped, and the analyst community was broadly constructive. JPMorgan raised its price target to $83 with an Overweight rating. Raymond James reiterated Strong Buy at $84. TD Cowen maintained Buy with a $78 target. The consensus across roughly 14 analysts landed around an $80 average target.
But the enthusiasm came with footnotes. Some analysts noted that Vera still faces launch execution risk, competition from other mechanism-based therapies in the pipeline, and the inevitable FDA scrutiny that comes with extraordinary claims. The stock gave back some initial gains as investors weighed those realities.
Trutakna received its first U.S. approval for reducing proteinuria in adults with primary IgAN at risk for progression, making it the only approved dual BAFF/APRIL inhibitor in this space. But it's entering a treatment landscape that's gotten a lot more interesting in recent years.
Sparsentan is already established as a disease-modifying option. SGLT2 inhibitors are now standard add-on therapy under international guidelines. Targeted-release budesonide (a steroid formulation designed to limit systemic side effects) remains available. And complement inhibitors, which target a different arm of the immune system, have also entered the market.
The pipeline is even busier: additional APRIL-targeting drugs, more complement inhibitors, and endothelin pathway agents like atrasentan (an endothelin A receptor antagonist being developed for CKD and proteinuria reduction) are all jockeying for position.
What separates Trutakna is its mechanism. It's the only approved therapy that directly targets the BAFF/APRIL axis, which means it attacks the disease at its immunological root rather than managing the downstream consequences. If the ORIGIN 3 data holds up in real-world use, it could become the anchor of combination regimens rather than just another option on the menu.
For decades, IgA nephropathy treatment felt like putting sandbags around a house during a flood: useful, but not exactly solving the problem. The shift toward mechanism-based therapies has changed the calculus, and Trutakna's data represents the most dramatic evidence yet that targeting the upstream immune drivers can genuinely alter the disease's trajectory.
