

Sarepta's gene therapy Elevidys just got the FDA's most severe safety warning for fatal liver failure, while the company cuts 36% of its workforce. A near-billion-dollar franchise is suddenly fighting for survival, and it raises hard questions about gene therapy's commercial future.
Imagine building a $900 million-a-year product, then watching the FDA slap its most severe safety warning on the label while you hand out 500 pink slips. That was Sarepta Therapeutics' week.
The company's flagship gene therapy, Elevidys (delandistrogene moxeparvovec), just got a boxed warning for acute serious liver injury and acute liver failure, including fatal outcomes. At the same time, Sarepta announced it's cutting roughly 500 employees, or 36% of its workforce, in a restructuring designed to save about $400 million a year. For a company that was supposed to be the poster child of rare disease gene therapy, this is a gut punch.
A boxed warning (often called a "black box") is the FDA's loudest alarm bell. It's the bold-bordered text at the very top of a drug's label that tells doctors: this can kill people if you're not careful. Plenty of drugs carry them and still sell well. But for a gene therapy that costs a fortune and treats children, it changes the conversation.
The trigger was fatal acute liver failure in non-ambulatory pediatric patients (kids who can no longer walk) treated with Elevidys. The FDA confirmed at least two fatal cases from clinical trials and post-marketing reports. In response, the agency didn't just add the warning; it pulled the non-ambulatory indication entirely. Elevidys is now approved only for ambulatory patients ages 4 and older with a confirmed DMD gene mutation.
The updated label recommends weekly liver function tests for at least three months after infusion. Patients with preexisting liver problems or active hepatic viral infections shouldn't receive it at all. Think of it like this: the FDA drew a much smaller circle around who can safely get this therapy, then put flashing red lights around the edges.
Elevidys had a blockbuster-caliber 2025, pulling in in net product revenue for the full year. But zoom in on the quarterly trend and the picture looks like a ski slope.

Q1 2025 was a monster: $375 million. By Q4, that number had cratered to $110.4 million. The culprit? Sarepta had already suspended shipments to non-ambulatory patients in the U.S. during the second half of the year as safety concerns mounted. Fewer eligible patients meant fewer infusions.
The 2026 numbers tell the rest of the story. First-half revenue came in at just $200.1 million, putting the drug on pace for roughly half of what it earned in 2025. When your top product loses nearly half its addressable market overnight, the math gets ugly fast.
Sarepta framed the layoffs as "strategic restructuring," but the calculus is straightforward: shrinking revenue plus mounting safety headaches equals a company that needs to get smaller to survive.
The $400 million in projected annual savings breaks down into two buckets. Headcount reductions account for about $120 million in annual cash savings. The bigger chunk, roughly $300 million, comes from pipeline reprioritization, which is corporate-speak for shelving or partnering out lower-priority gene therapy programs.
The company signaled it's pivoting toward its siRNA platform (a different technology that uses small RNA molecules to silence disease-causing genes) while pulling back on some gene therapy bets. Sarepta also noted the restructuring would involve $32 to $37 million in one-time severance charges. For 500 people, that works out to roughly $65,000 per person on average.
The subtext is hard to miss: Sarepta is preparing for a world where Elevidys is a much smaller product than anyone hoped.
You'd expect a black box warning and mass layoffs to crater a stock. And in normal circumstances, they would. But Sarepta's shares actually jumped 33% in after-hours trading after the restructuring announcement. Why?
Because investors were bracing for something worse: a full market withdrawal. Analysts at Evercore ISI noted that the restructuring eased fears that Elevidys might be pulled entirely for ambulatory patients. Jefferies called the label change "no surprises" and suggested it could even be a positive signal that the drug would stay on the market.
That said, the longer-term outlook is cautious. The bull case is that Sarepta stabilizes, keeps Elevidys on the market for ambulatory patients, and rebuilds around its siRNA pipeline. The bear case is that the deaths permanently damage the franchise, and the narrower label makes it nearly impossible to hit meaningful revenue targets.
Sarepta's troubles aren't just a Sarepta story. They're a stress test for the entire gene therapy commercial model.
The promise of gene therapy has always been elegant: one treatment, potentially for life. But the business model has a fundamental tension. You're asking insurers to pay millions of dollars upfront for a therapy whose long-term benefits are still being measured. When that therapy also carries a boxed warning for fatal liver failure, the sales pitch gets considerably harder.
Zolgensma (Novartis's gene therapy for spinal muscular atrophy) demonstrated that premium pricing can work, with a list price north of $2 million per patient in some markets. But it also showed that payer pushback, manufacturing constraints, and small patient populations create a ceiling that's hard to break through.
For Elevidys, the math has shifted. The addressable patient pool just shrank. Doctors will be more cautious (weekly liver tests for three months is a lot of monitoring). And payers now have a boxed warning to point to when negotiating coverage restrictions.
Sarepta isn't dead. It still has a marketed product generating meaningful revenue, a pipeline it's reorganizing around, and enough cost cuts to buy itself runway. The FDA is requiring a post-marketing observational study in 200 DMD patients to better define the liver risk, which could eventually help refine patient selection and restore some confidence.
But this is a company that entered 2025 with a near-billion-dollar gene therapy franchise and exited 2026's first half with half the revenue, a third fewer employees, and the most serious safety warning the FDA can issue. The road back is long, and it starts with proving that the patients who can still receive Elevidys will benefit safely.
For the gene therapy field, the lesson is uncomfortable but important: getting a drug approved is only the first act. Keeping it on the market, keeping patients safe, and keeping the business model intact? That's the whole play.
