

Pfizer just recalled 11 lots of the only drug approved to treat syphilis in pregnant women, and the supply was already in a three-year shortage. With congenital syphilis cases up 700% since 2015, the timing could not be worse.
Penicillin was discovered in 1928. Nearly a century later, it's still the only drug that can protect an unborn baby from syphilis. And right now, the sole U.S. manufacturer just yanked a bunch of it off the shelves.
Pfizer issued a voluntary recall of 11 lots of Bicillin L-A, its injectable penicillin G benzathine product, after finding particulates (tiny visible particles that shouldn't be there) during routine visual inspection. The company rated the health risk as "medium" and said it hadn't received any adverse event reports tied to the recalled lots.
Sounds manageable, right? Except for one enormous problem: the supply was already broken before this happened.
Bicillin L-A has been in a U.S. shortage since April 2023. That's more than three years of constrained supply, driven by syphilis rates climbing faster than Pfizer can fill syringes. The shortage isn't expected to resolve until the fourth quarter of 2027 at the earliest.
Now layer a recall on top of that timeline. The recalled lots span both the 1.2 million unit and 2.4 million unit presentations, which are the two dosage forms clinicians actually use. State health departments have warned that a near-term stockout is possible, with wholesalers already rationing limited quantities to providers.
Think of it like a town with one well during a drought. The well was already running low. Now someone found contamination in part of what's left.
For most adults with syphilis, there's a workaround. The CDC recommends doxycycline (a common antibiotic) as a supply-conserving alternative for men and non-pregnant women during the shortage. That helps free up penicillin for the patients who truly need it.
But pregnant women don't get a workaround. According to CDC guidelines, penicillin G is the only therapy with documented efficacy for treating syphilis during pregnancy. There is no acceptable substitute. If a pregnant patient is allergic to penicillin, the recommendation isn't to use a different drug; it's to desensitize her to penicillin and then give her penicillin anyway. That's how irreplaceable this drug is.

The reason is biological. Penicillin is the only antibiotic proven to reliably treat a fetal syphilis infection in utero. Without it, syphilis passes from mother to baby, causing what's called congenital syphilis: a preventable condition that can lead to stillbirth, bone deformities, and organ damage in newborns.
Congenital syphilis in the U.S. isn't some niche concern. It's a full-blown public health emergency that's been building for years.
The CDC reported nearly 4,000 cases of congenital syphilis in 2024, roughly 700% higher than in 2015. The maternal syphilis rate has climbed relentlessly: from 280.4 per 100,000 births in 2022 to 324.6 in 2023, then to 357.9 in 2024. That's a 28% increase in just two years.
Perhaps the most frustrating statistic: a CDC analysis found that nearly 90% of congenital syphilis cases in 2022 were linked to missed testing or treatment during pregnancy. These weren't mysterious failures. They were preventable ones. Mothers who didn't get screened early enough, or who got diagnosed but couldn't access the one drug that works.
Now imagine telling those patients that the drug they need has been recalled.
The structural fragility here is staggering. Pfizer is the only FDA-approved domestic manufacturer of benzathine penicillin G in the United States. In a global penicillin market worth roughly $11 billion, the most critical formulation for preventing a devastating birth defect depends on a single company's production line.
To patch the gap, the FDA has allowed temporary imports of foreign penicillin products, including Lentocilin and Extencilline, from overseas manufacturers. But "temporary imports" is the pharmaceutical equivalent of borrowing your neighbor's garden hose during a house fire. It helps, but it's not a real solution.
The upstream supply chain adds another layer of risk. Market analysts have flagged that the active pharmaceutical ingredient (API) for penicillin products is heavily sourced from China, creating geopolitical exposure on top of the manufacturing concentration.
The response has followed a predictable triage pattern. CDC-linked guidance urges jurisdictions to strongly consider reserving all remaining Bicillin L-A for pregnant patients and newborns with confirmed or suspected congenital syphilis. State and local health departments are actively helping clinics locate whatever supply exists.
Pfizer, for its part, has said it will ration limited inventory while working to restore production. The company also maintains a medical request process for patients with confirmed congenital syphilis or those at high risk.
But rationing a scarce resource doesn't create more of it. And experts have warned that this recall could erase recent progress in reducing congenital syphilis cases, potentially making a crisis that was already worsening even harder to contain.
This story isn't really about particulates in a syringe. It's about what happens when you let critical drug supply chains narrow to a single point of failure, then act surprised when that point fails.
The U.S. has known about rising syphilis rates for a decade. It's known about the Bicillin shortage for more than three years. The CDC's own data shows that screening and treatment gaps drive the vast majority of congenital syphilis cases. And yet the country still relies on one company, one factory, and one drug to protect thousands of pregnant women and their babies each year.
Penicillin is nearly 100 years old. It costs pennies to make. The fact that it can become unavailable in the wealthiest country on earth isn't a supply chain hiccup. It's a policy failure.
And right now, the patients paying for it are the ones who have zero alternatives.
