The recall covers 12 lot numbers across both the 1.2 million-unit and 2.4 million-unit presentations. Pfizer's own health hazard assessment rated the risk to patients as "medium," and the company says it hasn't received any reports of adverse events tied to the affected lots.

And it doesn't change the underlying fragility of relying on essentially one manufacturer for a drug this critical.
Most drug shortages are inconvenient. Doctors switch to an alternative. Patients deal with a slightly different formulation. Life goes on.
Syphilis in pregnancy is different. The CDC's treatment guidelines couldn't be clearer: "Parenteral penicillin G is the only therapy with documented efficacy for syphilis during pregnancy." If a pregnant patient is allergic to penicillin, the recommendation isn't to use something else. It's to desensitize them to penicillin and then give them penicillin anyway.
Doxycycline, the main alternative for non-pregnant adults, is classified as FDA Pregnancy Category D. Translation: it can harm the developing baby. Ceftriaxone? Not proven to prevent transmission to the fetus. There is, quite literally, no Plan B for pregnant patients.

The disparities are staggering. Among American Indian and Alaska Native women, the rate surged 52% in just two years. Among Black women, it rose 30%. Congenital syphilis (the disease passed from mother to child before or during birth) hit nearly 4,000 cases in 2024. That's the 12th consecutive year of increases, and roughly 700% higher than 2015 levels. The rate of growth has slowed recently, with cases rising only about 2-3% from 2023 to 2024 compared with double-digit annual jumps in prior years. But the absolute numbers remain at historic highs.
And here's the gut punch: congenital syphilis is almost entirely preventable. Screen pregnant women early. Treat them with penicillin. That's it. Studies suggest timely treatment is about 98% effective at preventing transmission. The problem isn't medical knowledge. It's access to the drug.
Public health agencies are now operating in full triage mode. The CDC's directive is blunt: reserve all available Bicillin L-A for pregnant patients and infants with congenital syphilis. Everyone else gets doxycycline (14 days for early syphilis, 28 days for late or unknown duration).
State health departments from Massachusetts to California to Maine have issued similar advisories. When local supply runs out, clinicians are directed to contact their state's STD program or use Pfizer's Medical Request Process, which is restricted to cases involving congenital syphilis or pregnancy.
But that process has drawn sharp criticism. The National Coalition of STD Directors has called it slow and administratively burdensome, describing documented cases where paperwork delays meant a pregnant patient delivered before treatment could be arranged. When you're racing against a biological clock (ideally, treatment should happen at least 30 days before delivery), every day of bureaucratic delay carries real consequences.
Infectious disease experts have been blunt in their assessments. The situation has been described as "really a tragedy, because it's totally preventable." The combination of rising syphilis rates, constrained drug supply, and complex access pathways creates what officials are calling a "perfect storm" for worsened outcomes.
But the deeper issue goes beyond Pfizer or any single recall. The U.S. has essentially concentrated manufacturing of a critical, irreplaceable antibiotic in the hands of one primary supplier. There's no robust stockpile. No redundant manufacturing capacity. No contingency plan that doesn't involve emergency imports from overseas.
Public health leaders are now pushing for stronger federal stewardship of critical drug supplies, investment in domestic manufacturing of benzathine penicillin G, and better coordination between Pfizer, the FDA, the CDC, and local health systems.
The optimistic scenario: Pfizer resolves its manufacturing issues, Lentocilin imports bridge the gap, and supply normalizes by late 2026 or early 2027. The broader syphilis epidemic, which showed some signs of slowing in 2024 (primary and secondary cases fell about 22% from 2023 to 2024), continues to bend downward.
The pessimistic scenario: supply recovery drags into 2027, maternal syphilis rates keep climbing, and the congenital syphilis numbers that had just started to plateau tick back up because pregnant women simply can't get the one drug they need.
