

The FDA just launched its first real-time clinical trial pilots with AstraZeneca and Amgen, streaming live safety and endpoint data to regulators as trials run. If this works, it could reshape how every drug gets developed.
Imagine running a clinical trial the way we've always done it: collect months of patient data, bundle it into a massive package, ship it to the FDA, then wait. It's a little like mailing your professor a semester's worth of homework on the last day of class and hoping for the best.
The FDA just said that era is ending.
On April 28, the agency announced it has launched two proof-of-concept studies under a new initiative called "real-time clinical trials." The idea is exactly what it sounds like: instead of waiting until a trial wraps up to hand regulators a fat binder of results, sponsors stream key data to the FDA as the trial runs. Safety signals, clinical endpoints, vital signs; all flowing to reviewers in something close to real time, through a cloud-based platform called Paradigm Health.
The two companies piloting this? AstraZeneca and Amgen.
AstraZeneca is running a Phase 2 study called TRAVERSE in treatment-naïve mantle cell lymphoma (a type of blood cancer) at MD Anderson Cancer Center and the University of Pennsylvania. The FDA says it has already received and validated real-time signals from this trial, proving the technical plumbing actually works.
Amgen, meanwhile, is running a Phase 1b trial called STREAM-SCLC in limited-stage small cell lung cancer. The drug being tested is tarlatamab (brand name IMDELLTRA), a bispecific T-cell engager, which is a fancy way of saying it grabs your immune cells and points them at cancer. Tarlatamab is already approved for extensive-stage small cell lung cancer, where a Phase 3 trial showed it cut the risk of death by 40%. Now Amgen is pushing it into earlier-stage disease, with the FDA watching every data point come in live.
This effort sits inside a bigger HHS program called Operation TrialBlazer, launched on June 22, 2026. The motivation is blunt: the U.S. is losing its edge. Getting from a pre-IND meeting (the first formal chat with regulators) to enrolling the first patient in a trial can take in the U.S. Other countries do it in months.

The FDA estimates these changes could save sponsors 6 to 12 months on early-stage development alone. Some commentators think the overall impact could eventually cut U.S. trial timelines in half, though that's more aspiration than proven fact at this point.
Let's be precise about what's happening here, because "real-time" can mean a lot of things.
In the STREAM-SCLC trial, the FDA is specifically watching for safety signals like cytokine release syndrome (CRS), a potentially dangerous immune overreaction that can happen with drugs like tarlatamab. Instead of learning about a CRS cluster weeks or months after the fact, reviewers can see vital signs and adverse events as they're recorded. Think of it as going from getting a monthly bank statement to having a live transaction feed.
For the TRAVERSE trial, AstraZeneca and the FDA agreed in advance on exactly which signals would be reported in real time and what the criteria would be. The data flows through Paradigm Health's platform, and the FDA confirmed it can receive and validate those signals successfully.
This doesn't mean FDA staffers are sitting in a control room watching heart monitors blink. The model is closer to a shared dashboard: structured, pre-agreed data points get pushed to the cloud, where reviewers can access them continuously rather than in batch deliveries.
The real-time trials are just the tip of a much larger modernization iceberg. The FDA has spent years building the scaffolding for this moment: guidance on digital health technologies for remote data capture, frameworks for decentralized trials (where patients participate from home instead of trekking to a site), and modernized Good Clinical Practice standards that emphasize risk-based monitoring.
Operation TrialBlazer ties all of these threads together. The rolling IND platform alone could fundamentally change how sponsors interact with the agency. Today, you assemble your entire application, submit it, and then a 30-day review clock starts. Under the new model, you'd send sections as they're ready, get feedback iteratively, and submit the final package only after earlier pieces have already been reviewed. It's the difference between building a house and getting it inspected all at once versus having the inspector check the foundation before you pour the second floor.
The FDA also issued a Request for Information seeking public input on how a broader real-time trial pilot should be designed, with final selection criteria expected in July and pilot selections targeted for August.
Some caution is warranted. Two proof-of-concept studies at elite academic centers don't prove this model works at scale. MD Anderson and Penn are not your average community oncology practice. The real test will be whether smaller sites and mid-tier sponsors can plug into a cloud-based data streaming architecture without drowning in technical and compliance costs.
There's also the question of what the FDA does with all this real-time data. Faster information is only useful if reviewers have the bandwidth and tools to act on it. If the data streams in but decisions don't speed up accordingly, you've just added complexity without shortening timelines.
And let's not forget the regulatory politics. Operation TrialBlazer is framed partly as a competitiveness play against China. That framing helps secure political support today, but it also means the initiative's survival depends on whether future administrations share that priority.
The FDA just took its first real steps toward watching clinical trials unfold in real time, not reading about them after the fact. AstraZeneca and Amgen are the guinea pigs. If the model works, it could compress drug development timelines across the industry and fundamentally change the relationship between sponsors and regulators.
